Overview

HJF is seeking a bilingual Spanish-English Health Coach I to assess client health and educate on activities and regimens that promote a healthy lifestyle for Hispanic/Latinx patients with type 2 diabetes. The Health Coach will assist in the development and coordination of patient health and wellness through the development of self-management strategies in support of sustainable, healthy lifestyles.

This part-time (20 hours/week) position will support work at the Brooke Army Medical Center (BAMC) Diabetes Center of Excellence (DCOE) supporting the study: Supporting Access for Latinx Underserved in Diabetes Management (SALUD-M): An Equity-Driven Randomized Controlled Trial of Acceptance Based Coping Skills for Latinx Military Patients with T2DM.

The Henry M. Jackson Foundation for the Advancement of Military Medicine (HJF) is a nonprofit organization dedicated to advancing military medicine. Responsibilities
    Deliver the Acceptance Based Coping (ABaCo) skills program, a 10-week virtually-delivered educational program to enhance self-management, to military beneficiaries with type 2 diabetes.
  • Attend required initial training and ongoing weekly consultation to reach competence in delivering ABaCo.
  • Identify client/patient needs through a comprehensive assessment and analysis, including identification of behavioral, social, and environmental factors that could impact coaching and supportive needs of the client/patient.
  • Develop an individualized plan of care based on client/patient health status, background, and supportive needs to achieve goals for the client seeking assistance in all areas of health promotion. It may require working with both patient and family members/caregivers.
  • Monitor progress and follow-up with client/patient related to established goals. Utilize appropriate health coaching techniques to elicit behavior change using motivational interviewing, assessing patients' desires and motivation to change, and providing education appropriate to the patients' level of activation and literacy-document health coaching interactions.
  • Assist in special projects and studies. Aide in collecting and analyzing data that will help determine the impact of the integrative health coaching model on military client population health.
  • Collaborate with an interdisciplinary team of healthcare members to develop health promotional education programs. Conduct workshops, seminars, and other meetings with staff to educate them about integrative health coaching principles.
  • May perform other duties and responsibilities as assigned or directed by the supervisor. This may include attendance of and participation in required military and in-person training for the role.
Qualifications

Education and Experience

  • Bachelor's Degree in a health field (e.g., Health, Nutrition, Physiology, Public Health) or a behavioral science field (e.g., Psychology, Social Work, Human Services) is required.
  • Minimum of 0-2 years experience required.
  • Experience working with military population preferred.

Required Knowledge, Skills and Abilities

  • Working knowledge of military and community healthcare.
  • Fluent in Spanish and English.
  • Ability to teach diabetes self-management skills.
  • Knowledge of general healthcare practices, office workflows, and medical terminology.
  • Ability to teach groups and individuals, ability to work independently as well as collaboratively and in a team.
  • Knowledgeable in risk reduction factors in multi-symptom illness.
  • Knowledge of multi-symptom illness management through lifestyle change applications.
  • Strong verbal and written communication and interpersonal skills.
  • Computer skills and is proficient with Microsoft package.
  • Ability to write manuscrips is highly preferred.
  • US Citizenship and ability to obtain and maintain a T1/Public Trust background check.

Physical Capabilities

  • Lifting: Requires lifting materials up to 10 lbs.
  • Ability to stand or sit at a computer for prolonged periods.

Licenses and Certifications

  • Health Coaching certification highly preferred.

Work Environment

  • This position will take place primarily in a remote/virtual environment. Occasional in person activities required at Brooke Army Medical Center, Fort Sam Houston, TX include: in-processing, obtaining equipment, joining maintenance visits as needed for computer equipment.

Some positions or sites may require that the incumbent be fully vaccinated against COVID-19. Proof of vaccination may be required.
